RMA Request - Interscan Corporation

Please complete the Return Material Authorization (RMA) form below, and click on the submit button at the bottom of the form. A copy of the completed form and the RMA number will be e-mailed back to you.
All service and repairs are payable by credit card or check for USA orders or credit card or wire transfer for international orders. Wire transfers are subject to a $75.00 export fee/handling fee.
Interscan's warranty is one (1) year from date of shipment for instruments, and six (6) months from date of shipment for sensors and furnaces.
Ship all units approved for return, with RMA number marked on outside of container.
NOTE: Customer will be notified if any items are left, unclaimed, with Interscan for more than three (3) months. Should Interscan receive no response, ownership of such items shall be relinquished to Interscan, and the said item(s) shall be disposed of at that time.

NOTE: When returning sensors for ECS, please provide the serial number of the instrument in which the sensor will be used. This is required to ensure that the adjustments we send to you are accurate.

ECS charges (for sensor only) are $400 for all gases except hydrazine; $500 for hydrazine. If a full instrument is returned for calibration service, the charge is $400 for all gases except hydrazine; $500 for hydrazine. Please note that additional issues with your instrument may be diagnosed, and would be subject to additional charges. You will be advised of all charges, upon evaluation of your returned equipment.
If your sensor fails calibration do we have your permission to replace the sensor at estimated cost of $875?

All repaired items destined for the United States are shipped via UPS Ground, freight charges to be prepaid and added to invoice---unless your carrier and account number have been provided. If you prefer another method of shipment or are located outside the US, please state carrier and account number.

Defective parts will be disposed of by Interscan, unless we are instructed otherwise by the customer.
All materials (i.e. individual sensors or gas analyzers) are subject to a $300.00 evaluation charge per each item--whether or not Interscan has received authorization to proceed with repairs. The $300.00 evaluation charge will be applied to the service/repair cost.
Interscan Corporation is NOT responsible for any customs duties, import taxes, or fees that may occur in the returned goods/service transaction. All fees are the sole responsibility of the consignee or the customer who purchased the instrumentation, parts, or service.
Interscan Corporation will advise the shipping company and/or customs officials to abandon the shipment if the customer/consignee refuses to pay the customs duties, taxes, or fees. The customer/consignee will NOT be reimbursed for any funds paid to Interscan, nor will Interscan Corporation be responsible for replacement of the abandoned items.
